If you would like, you can print them and fill them out now, then either bring them to your first session, scan and email them to us, or mail them to us physically.
This will allow us skip the forms during our intake session and immediately start talking about how we can work together to help you.
This form gives us all of your demographic information as well as your insurance information.
This form gives us permission to provide therapy to you, and informs you about some of our policies. It also gives you a place to let us know how you would prefer to be contacted, and whether we can leave messages, etc.
This does not need to be signed, but we are required to provide you with a form that discusses our privacy policy.
Because of confidentiality, we will not let anyone know that you are in therapy. (See Informed Consent form for more information about confidentiality). However, there are some times when we will need to be able to talk with a family member, get medical records, and/or send insurance claims. We will need at least one of these consents - for your insurance. Others to consider are partners/spouses, parents, emergency contacts, previous therapists, etc.
We take credit card information to allow you to use your card to pay for services. This form allows us to do so without discussing it at every session. It also gives us permission to charge you for 'no-shows'.
